Job Summary

We are seeking a highly skilled Registered Nurse Home Health to join our elite team of healthcare professionals. As a Registered Nurse Home Health, you will play a vital role in providing high-quality care to our patients in their preferred home setting.

Responsibilities
  • Assess patient needs and develop individualized plans of care to ensure optimal health outcomes
  • Provide direct patient care services, including wound care, medication management, and education
  • Collaborate with interdisciplinary teams to coordinate patient care and services
  • Maintain accurate and detailed records of patient interactions and care provided
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
Requirements
  • CURRENT, UNRESTRICTED STATE RN LICENSEE required
  • Minimum 1+ year experience in clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
  • Associates Degree or Bachelor's Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
