Job Summary

This role involves developing and delivering comprehensive education programs, evaluating patient responses to therapeutic interventions, and staying up-to-date with the latest trends in diabetes care.

Key Responsibilities
  • Develop and deliver diabetes education programs for patients, families, and caregivers, including new onset education, self-management, and advanced pump classes.
  • Evaluate and update education materials and teaching methods to ensure effectiveness and relevance.
  • Monitor and evaluate patient responses to therapeutic interventions, collaborating with providers to implement treatment plans.
  • Provide nutritional and exercise counseling for weight control and disease management.
  • Manage urgent diabetes care issues in collaboration with providers.
  • Develop relationships with other departments and community health care agencies to support quality diabetes care.
  • Participate in training and education of nursing staff in diabetes and related clinical procedures.
  • Maintain professional growth and development through research, seminars, and professional affiliations.
Requirements
  • Bachelor of Science in Nursing or equivalent degree from an accredited institution.
  • Current licensure as a RN in the State of IL or Registered Dietitian with the Commission on Dietetic Registration.
  • CPR certification per American Heart Association guidelines.
  • Current CDE certification through the National Certification Board of Diabetes Educators.
Working Environment

This role is based in an indoor office environment or hospital setting, with a sitting-to-standing ratio of 80:20.
