We are currently seeking a skilled Registered Nurse (RN) Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating client care, performing visits, and overseeing the frequency of visits for each episode.

You will have the chance to work closely with our multidisciplinary team, including physicians, therapists, and social workers, to provide comprehensive care to our patients.
The ideal candidate will possess excellent communication skills, both written and verbal, and be able to effectively communicate with patients, families, and healthcare professionals.
Responsibilities- Coordinate client care and perform visits as needed
- Oversee the frequency of visits for each episode
- Consult with physicians and other healthcare professionals as needed
- Perform accurate OASIS collection and ensure medication profiles are up-to-date
- Current licensure as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
We offer a competitive salary range of $83,000 - $110,000 per annum, depending on experience. In addition, we provide a range of benefits, including:
