Job Summary:

We are seeking an experienced Speech Therapist to join our team. As a Speech Therapist, you will play a vital role in facilitating the rehabilitation of patients with emotional, physical, or mental disabilities.

Key Responsibilities:

  • Develop and implement individualized speech therapy care plans to meet the unique needs of each patient.
  • Collaborate with our multidisciplinary team to ensure comprehensive care and coordination.
  • Provide education and support to patients and their families to promote optimal outcomes.
  • Document patient progress and maintain accurate clinical records.

Requirements:

  • Current licensure as a Speech Language Pathologist in the State of Texas.
  • Experience in home health or a related field is highly preferred.
  • Proficiency in Home Care Home Base EMR is a plus.
  • Knowledge of home health regulations and general healthcare compliance.
  • Ability to handle physical workload and maintain a safe work environment.
  • Good driving record and reliable transportation.
  • Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
  • Exceptional communication and interpersonal skills.
