Key Responsibilities:- Establish and implement client service plans in coordination with our staff.
- Provide skilled nursing care and services on an hourly or per visit basis.
- Regularly assess the nursing needs of our clients and conduct initial face-to-face assessments.
- Work with clients and their families to determine their needs and provide education on proper care and treatment.
- Supervise and train our Certified Nursing Assistants (CNAs) and other home health aides.
- Perform supervisory visits to ensure high-quality care and make recommendations for improvement.
- Collaborate with our physicians to revise client plans and ensure accurate and complete documentation.
- Assist with the orientation of new caregivers and conduct annual evaluations.
- Current RN licensure and a strong background in nursing care.
- Excellent communication and leadership skills.
- Ability to work independently and as part of a team.
- Strong attention to detail and ability to maintain accurate records.
