Case Manager III

We are seeking a highly skilled and compassionate Case Manager III to join our team at Urban Strategies. As a key member of our Post Release Services team, you will be responsible for providing intensive case management services to children and their sponsors, ensuring their safety and well-being in a crisis.
Key Responsibilities- Manage an assigned caseload of post release services, providing trauma-informed care and support to children and their sponsors.
- Conduct home visits, interviews, and assessments to identify and address the needs of children and their sponsors.
- Develop and implement safety plans, providing crisis intervention services and connecting families to resources and services.
- Collaborate with children, sponsors, and external agencies to ensure the safety and well-being of children and families.
- Maintain accurate and confidential records, adhering to program regulations and guidelines.
- Participate in training, meetings, and quality improvement initiatives to ensure the highest level of service delivery.
- Masters in Social Work (MSW) or equivalent degree in psychology, sociology, or other behavioral science or social service field.
- Minimum 5 years of clinical experience, with a focus on trauma-informed care and child welfare practices.
- Experience working with immigrant populations and providing post-release services.
- Knowledge of family preservation, kinship care, trauma-informed practices, child welfare policies, and procedures.
- Effective communication and interpersonal skills, with the ability to work with diverse populations.
- Fluent in English and Spanish (verbal/written), with proficiency in Microsoft Office and data entry systems.
