Job Summary

We are seeking a highly skilled OB-GYN Nurse Navigator to join our team at Chi Saint Joseph Medical Group. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring seamless coordination of services.

Responsibilities
  • Provide comprehensive nursing care and guidance to patients throughout their care continuum, ensuring timely and effective communication with all healthcare providers.
  • Systematically assess, plan, implement, and evaluate patient care according to ACOG and Chi Saint Joseph Health standards of practice.
  • Facilitate coordination of patient care services to ensure excellence in patient care and patient flow, eliminating operational barriers to service.
  • Interface with other healthcare teams for referrals and services, including social work, pastoral care, and community services.
  • Initiate and document patient teaching, including family and significant others, based on assessment of needs, and respond to patient requests for information regarding the disease process and expected side effects of treatment.
  • Track and communicate monthly referrals by disease site and distribution as requested.
  • Continuously assess patient progress and educational needs, making adjustments to care based on assessment, and consult with primary physician or designee and other members of the care delivery team as needed.
Requirements
  • Minimum of 5 years of experience in inpatient or outpatient OBGYN setting, or related experience required.
  • KY RN licensure required.
  • ADN or BSN in nursing required.
Preferred Qualifications
  • Experience/knowledge in community health setting or services.
