Job Summary

We are seeking a highly skilled Care Management Informatics Specialist to join our team at LifeBridge Health. As a key member of our care management team, you will be responsible for leading the integration of care management functions, computer, and information sciences to manage and communicate data, information, and knowledge throughout the care management continuum.

Key Responsibilities
  • CM Informatics: Utilize leadership skills to integrate care management functions, computer, and information sciences to manage and communicate data, information, and knowledge throughout the care management continuum.
  • Operations: Ensure effective operations of care management service areas.
  • Evidenced-Based Practice: Assist in the integration of the best available evidence into organizational policies and procedures.
  • CM Technical Support: Serve as a technical and operational workflow support for purchased software products utilized by the care management and utilization management teams.
  • Training: Develop and oversee training of personnel for new or modified software and specialty systems.
Requirements
  • Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline
  • 5-7 years related experience
  • Maryland Registered Nurse License
  • American Heart Association CPR Certification
  • Professional Organization Participation - required
  • Informatics Certification - required
