LPN Job Description

We are seeking a skilled LPN to join our team at Providence Medford Medical Center. As a Licensed Practical Nurse, you will play a vital role in delivering high-quality patient care and contributing to a positive patient experience.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with the healthcare team to develop and implement individualized patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Graduate of an approved school for practical nurses.
  • Oregon Licensed Practical Nurse.
  • National Provider BLS - American Heart Association.
  • 1 year LPN Experience.
Preferred Qualifications:
  • IV Therapy course and competency demonstration.
