Job Summary:

Maxim Healthcare Services is seeking a Licensed Practical Nurse (LPN/LVN) to join our team in providing high-quality care to our patients in the comfort of their own homes. As a LPN/LVN, you will be responsible for assessing, planning, implementing, and evaluating patient care, while working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.

Responsibilities:

* Utilize the nursing process to assess, plan, implement, and evaluate patient care
* Perform focused interviews to identify specific patient needs
* Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
* Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses

Requirements:

* Current Licensed Practical Nurse (LPN/LVN) in the state in which the LPN/LVN practices
* Current Health Certificate (per facility)
* Current PPD or Chest X-Ray
* Current BLS card
* One year prior LPN/LVN experience preferred
