Key Responsibilities
  • Provide high-quality, patient-centered care to individuals with serious illnesses
  • Conduct thorough assessments, develop care plans, and implement interventions to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination
  • Supervise and mentor Hospice LPNs and C.N.A.s, as assigned
  • Participate in ongoing education and training to stay current on best practices and evidence-based care
Requirements
  • Graduate of an accredited Registered Nurse Program
  • One year of experience in hospice, home health, oncology, or medical surgical nursing
  • Current licensure as a Registered Nurse in the state of Virginia or approved compact state
  • Valid driver's license, automobile insurance, and personal vehicle for making home visits
  • AHA BLS-HCP certification
  • Clear criminal records check and successful completion of orientation and competency requirements
