Community Care, Inc. is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality in-home care to adult patients in the Monongahela area. As a member of our team, you will work under the guidance of a Registered Nurse (RN) to deliver direct nursing care and support patients in their homes.
Key Responsibilities- Implement and evaluate patient care activities designated by the RN case manager or RN supervisor.
- Develop and implement nursing care plans under the direction of the RN, making suggestions as needed to ensure patient safety and well-being.
- Administer prescribed medications and perform specific treatments as ordered by physicians under RN supervision.
- Make observations related to patient conditions, home situations, and treatment responses.
- Accept verbal physician orders after policy orientation and within regulatory and agency policies.
- Complete electronic shift notes and nursing visit notes, documenting treatment provided and patient responses.
- Submit clinical notes timely using an electronic medical record.
- Report observations of patient clinical changes, treatment responses, and home situations to the supervising RN and/or case manager.
- Maintain confidentiality of client information adhering to HIPAA regulations.
- Maintain a professional standard of conduct and project a positive image of the agency at all times.
- Comply with infection control and safety policies and procedures.
- Adhere to all policies and practices of the agency to maintain established Standards for Care.
- Adhere to all policies set forth in the Nurse Practice Act to maintain safe clinical practices.
- Graduate of a state-approved school with a course in pharmacology.
- Current license in the State of Pennsylvania.
- Minimum of six (6) months experience in relevant patient care.
- Current CPR Card.
- Reliable independent means of transportation required for travel between assignments.
- Clear criminal history, Childline Clearance, FBI Clearance, DIG, Medicheck, EPLS, and any additional required regulatory clearances.
