About the Role
We are seeking a skilled and compassionate VNA Nurse - LPN to join our team. As a VNA Nurse - LPN, you will have the opportunity to make a meaningful difference in the lives of our clients, providing vital care and support in the comfort of their own homes.
Responsibilities
- Administer medications and provide vital care to clients
- Conduct thorough assessments and develop personalized care plans
- Work independently and as part of a team to deliver exceptional care
- Communicate effectively with clients, families, and healthcare professionals
- Maintain accurate records and documentation
Requirements
- Valid Connecticut RN or LPN license
- Up-to-date CPR certification
- Strong assessment skills and ability to work independently
- A reliable vehicle and a valid driver's license
- Compassionate demeanor and patient-centered approach to care
- Fluency in English and Spanish is a plus
