As a Clinical Pharmacist in our Hemostasis and Thrombosis program, you will play a key role in providing high-quality direct patient care through collaborative drug therapy management. You will work closely with our primary care pharmacists and inpatient pharmacists to support the transitions of care for clinic patients.
Key Responsibilities- Provide medication therapy for anticoagulant, antiplatelet, and hemostatic agents
- Collaborate with healthcare teams to develop and implement individualized treatment plans
- Monitor patient outcomes and adjust treatment plans as needed
- Stay up-to-date with the latest research and guidelines in hemostatic and thrombotic disorders
- Pharm.D. or B.S. in Pharmacy from an accredited College of Pharmacy
- Eligible for pharmacist licensure through the Board of Pharmacy in the State of Maine
- PGY1 Pharmacy residency strongly preferred
- Ambulatory care experience with direct patient care preferred
