We are seeking a skilled and experienced Case Management RN to join our team at Travel Nurses, Inc. in Gainesville, Florida. This is a 13-week assignment with 36 hours per week, 12-hour shifts, and a competitive pay package.

About the Job

This is a unique opportunity to work with a leading travel nurse staffing agency, providing high-quality care to patients in a dynamic and fast-paced environment. As a Case Management RN, you will be responsible for coordinating patient care, developing treatment plans, and collaborating with healthcare teams to ensure optimal patient outcomes.

Responsibilities
  • Coordinate patient care and develop treatment plans in collaboration with healthcare teams
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions
  • Develop and implement effective care plans to achieve optimal patient outcomes
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with healthcare teams, patients, and families to ensure timely and effective care
Requirements
  • 1-2 years of recent experience in Case Management
  • Current RN licensure in the state of Florida
  • Ability to work 36 hours per week, 12-hour shifts
  • Strong communication and collaboration skills
  • Ability to work in a fast-paced environment
