Nurse Practitioner or Physician Assistant

About the Role:
Hartford HealthCare (HHC) is seeking an experienced per diem Nurse Practitioner or Physician Assistant to join our REACH (Resource for Elderly And Care at Home) Program, serving geriatric patients in the greater Fairfield area.
Responsibilities:
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
Requirements:
- Unrestricted State of Connecticut Medical License (or eligible)
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training
- At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
