Job SummaryWe are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. Responsibilities- Provide primary care to patients in their homes, including routine check-ups, medication management, and health education.
- Address urgent care needs, such as acute illnesses, injuries, and chronic condition management.
- Collaborate with our interdisciplinary team, including physicians, social workers, and care coordinators, to ensure seamless care coordination.
- Develop and implement individualized care plans, taking into account patients' unique needs, preferences, and goals.
- Conduct initial patient screenings, physical examinations, and create appropriate assessments and plans.
- Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications, including psychopharmacological agents, as appropriate.
- Update patient records and ensure accuracy at each patient appointment.
- Deliver palliative care and end-of-life care to patients and their families, as needed.
- Facilitate family meetings with our care team to address complicated family dynamics and goals of care.
- Master's degree in Nursing.
- Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
- 5+ years of progressive care experience.
- BLS and ACLS certification, and an NPI# as an NP.
- Valid driver's license, car insurance, access to a car, and willingness to drive to patient homes/patient locations.
