Job Summary

We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to seniors in their homes. Your expertise will be essential in delivering comprehensive care, addressing complex medical conditions, and improving patient outcomes.

Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, medication management, and health education.
  • Address urgent care needs, such as acute illnesses, injuries, and chronic condition management.
  • Collaborate with our interdisciplinary team, including physicians, social workers, and care coordinators, to ensure seamless care coordination.
  • Develop and implement individualized care plans, taking into account patients' unique needs, preferences, and goals.
  • Conduct initial patient screenings, physical examinations, and create appropriate assessments and plans.
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications, including psychopharmacological agents, as appropriate.
  • Update patient records and ensure accuracy at each patient appointment.
  • Deliver palliative care and end-of-life care to patients and their families, as needed.
  • Facilitate family meetings with our care team to address complicated family dynamics and goals of care.
Requirements
  • Master's degree in Nursing.
  • Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
  • 5+ years of progressive care experience.
  • BLS and ACLS certification, and an NPI# as an NP.
  • Valid driver's license, car insurance, access to a car, and willingness to drive to patient homes/patient locations.
