We are seeking a skilled Physical Therapist to join our team at Meadow Lakes, a part of American Senior Communities. As a Physical Therapist, you will play a vital role in delivering patient-centered care to our residents.

Key Responsibilities
  • Implement skilled physical therapy services for patients with minimal supervision, adhering to the principles and practices of physical therapy and ASC Therapies and Wellness policies and procedures.
  • Screen and evaluate patients to determine their skilled therapy needs and develop initial and ongoing care plans.
  • Collaborate with caregivers and other healthcare professionals to ensure comprehensive care.
Requirements
  • Graduate of an American Physical Therapy Association (APTA) accredited school of physical therapy.
  • Indiana licensure or ability to obtain Indiana licensure.
