Job Summary

Summit Home Care is seeking a compassionate and reliable Home Health Aide to join our team. As a Home Health Aide, you will provide personal care and support to patients in their homes, promoting their independence and well-being.

Responsibilities
  • Provide daily personal care, including bathing, grooming, oral care, and dressing
  • Assist with daily living activities as outlined in the patient's Plan of Care
  • Monitor patients' conditions and report any changes to the healthcare team
  • Assist with patient meals and feeding, as well as basic housekeeping functions
  • Accompany patients to doctor's visits, shopping, and other essential errands
Requirements
  • New York State Home Health Aide Certificate
  • Government-issued ID, Social Security Card, and Work Authorization (if required)
  • Two reference letters
  • Recent physical examination
  • TB screening (Quantiferon preferred)
  • Flu shot
  • Rubella and Rubeola titer lab work
  • Chain of Custody Drug Screen (no older than 6 months)
