Hebrew SeniorLife is seeking a highly skilled Geriatric Specialist to join our team. As a Geriatric Specialist, you will be responsible for providing high-quality medical care to seniors in our long-term chronic care, community, and home settings.
Key Responsibilities:- Perform medical examinations, evaluations, assessments, diagnoses, and treatment to long-term chronic care patients, post-acute care inpatients, and/or outpatients.
- Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists and to relevant patient care components as appropriate.
- Directs and coordinates the patient care activities of clinical team members and support staff as required.
- Coordinates with members of a multidisciplinary team to help meet each patient's individual needs.
- MD or DO degree; completion of an internal medicine or family practice residency (minimum three years) and completion of a geriatric medicine fellowship.
- Valid and unrestricted Massachusetts Medical License.
- Board Certified/Board Eligible in Internal Medicine or Family Medicine or equivalent; Board Certified/Board Eligible in Geriatric Medicine or equivalent.
- Federal DEA Certification and CPR/BLS/ACLS as required.
