At Choices Healthcare, we are seeking a skilled Licensed Practical Nurse to join our Home Hospice team. As a LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to address the unique needs of our patients and their families.

Key Responsibilities:

  • Provide personal care services and skilled nursing services to patients and caregivers
  • Collaborate with the Interdisciplinary Team to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

Requirements:

  • Minimum 1 year of LPN experience required
  • Ability to work a minimum of 64 hours per two-week pay period
  • Must be vaccinated for influenza or be granted a religious/medical exemption
