Job Summary:We are looking for a skilled and experienced Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will have a strong background in patient assessment, care planning, and medication management. If you are a motivated and compassionate nurse who is passionate about delivering exceptional patient care, we encourage you to apply.
Responsibilities:- Provide skilled nursing care to patients in their homes, including assessment, care planning, and medication management.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Document patient progress and care provided in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current, unrestricted state license as a Registered Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have a strong sense of smell and touch.
- Must be able to sufficiently reposition patients and move equipment without assistance.
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
