Job Summary

We are seeking a highly skilled Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will provide comprehensive skilled services to patients in their homes, promoting optimal recovery and rehabilitation.

Key Responsibilities
  • Plan of Care: Develop and implement individualized plans of care for patients, incorporating physical therapy interventions to address functional limitations and promote independence.
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Minimize Patient Risk: Identify and mitigate risks associated with patient care, including falls, skin breakdown, and other potential complications.
  • Family Educator/Advocate: Educate patients and their families on disease management, prevention, and self-care strategies, promoting empowerment and independence.
  • Supervision: Supervise and mentor physical therapy assistants and other healthcare professionals as needed, ensuring high-quality patient care.
  • Communication/Collaboration: Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Policies and Procedures: Adhere to agency policies, procedures, and regulatory requirements, ensuring compliance and maintaining high standards of patient care.
Requirements
  • Education and Licensure: Current licensure as a Physical Therapist in the state of employment.
  • Experience: Minimum 1 year of experience in home health or a related setting.
  • Skills and Abilities: Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
