Job Title: Case Manager Registered Nurse

We are seeking a skilled Case Manager Registered Nurse to join our team at the University of New Mexico. As a Case Manager RN, you will play a critical role in coordinating patient care and ensuring quality, cost-effective outcomes.

Job Summary:

The Case Manager Registered Nurse will be responsible for coordinating all systems and services required for an organized, multidisciplinary, patient-centered care team approach. This includes managing the course of treatment of patients, coordinating care with physicians, nurses, and other staff to achieve quality patient outcomes within established time frames and with efficient utilization of resources.

Key Responsibilities:
  • Conduct initial and ongoing assessments to determine patient needs and develop comprehensive care plans.
  • Coordinate care with physicians, nurses, and other staff to ensure quality patient outcomes.
  • Manage the course of treatment of patients, including initiating disease management protocols and determining and managing outcomes.
  • Ensure continuity of care through discharge planning, utilization of resources, and analysis of variances.
  • Function as a contact person for patients, families, healthcare team members, community resources, and employees as necessary.
Requirements:
  • 2 years of directly related experience as a Registered Nurse.
  • RN licensure in New Mexico or reciprocal agreement.
  • CPR for Healthcare/BLS Provider or Professional Rescuer certification within 30 days.
Preferred Qualifications:
  • Bachelor's Degree in Nursing.
  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo.
Working Conditions:

This is a light work position that requires exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly. The position may require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
