Job Summary

US Renal Care is seeking a highly skilled Dialysis Registered Nurse to join our team as a Home Hemodialysis Specialist. As a key member of our home dialysis program, you will be responsible for providing exceptional patient care and training to patients choosing a home dialysis modality.

Key Responsibilities
  • Train patients and their care partners in the practice of self-care peritoneal dialysis or hemodialysis, ensuring compliance with federal and state regulations.
  • Conduct home visits to assess the patient's home environment and evaluate patient performance, making recommendations for improvement as needed.
  • Communicate effectively with patients, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Document all nursing services in the Electronic Medical Record, accurately reflecting patient status and nursing interventions.
  • Participate in infection control monitoring, implementation, and recording, and be familiar with emergency equipment and procedures.
Requirements
  • Current RN licensure and experience in home dialysis or critical care.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with a focus on patient-centered care.
