- Coordinate Services: Work with community health resources to plan and direct services, ensuring seamless coordination of care and effective communication with patients, families, and healthcare providers.
- Provide Direct Care: Administer medications and treatments as prescribed by physicians, provide direct patient care, and counsel patients and families on meeting their nursing and related needs.
- Monitor and Evaluate: Regularly re-evaluate patient nursing needs, update care plans as necessary, and ensure that patients receive high-quality care that meets their evolving needs.
- Collaborate with Team Members: Work collaboratively with other healthcare professionals, including physicians, therapists, and home health aides, to ensure comprehensive and coordinated care.
- Education and Licensure: Graduate of an accredited school of nursing, current licensure in state, and CPR certification.
- Experience: One to two years of recent acute care experience in an institutional setting, with home health care experience preferred.
- Skills and Abilities: Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
