Home Health RN Job Description

We are seeking a skilled and compassionate Registered Nurse to join our Home Health Care team at Providence Benedictine Nursing Center. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities:
  • Provide direct and indirect quality care for patients of all ages, following the nursing process and in accordance with the Nurse Practice Act.
  • Coordinate patient care activities with other members of the health care team in a collegial relationship.
  • Develop and implement individualized care plans, ensuring patient-centered care and promoting optimal health outcomes.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions and effective communication.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Administer medications, treatments, and therapies as prescribed, ensuring patient safety and comfort.
  • Monitor patient progress, adjust care plans as needed, and document all interactions and interventions.
Requirements:
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • 1 year of Medical/Surgical experience in an acute care setting.
  • 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
Preferred Qualifications:
  • Recent Home Health experience.
  • Hospice or Palliative Care experience for Palliative Care RN work assignment.
  • Point of Care experience.
