About the Role:
The ideal candidate will have 3 years of clinical experience in acute care, rehab, home health, or long-term care. You will be responsible for:
- Managing healthcare needs and patient outcomes
- Coordinating services and resources using a team approach
- Evaluating and planning patient care during hospitalization
- Facilitating post-hospital care arrangements
- Contributes to performance improvement initiatives
What You Bring:
To be successful in this role, you will need:
- Experience in admission and discharge risk assessment
- Proficiency in utilization management
- Knowledge of CMS, Kepro QIO, and healthcare regulations
Education and Training:
A Masters in Social Work is required for this position.
License, Certification & Registration:
LICSW is preferred, but LCSW is accepted.
