Job Summary:We are looking for a dedicated and compassionate Speech Language Pathologist to work with our multidisciplinary healthcare team in delivering rehabilitative care and skilled nursing services to clients in the comfort of their own homes.
Responsibilities:- Conduct thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
- Develop and implement individualized treatment plans based on client needs and physician orders.
- Collaborate with the multidisciplinary care team to ensure comprehensive care and coordination.
- Educate and instruct patients, family members, or caregivers on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations in client records.
- Current license as a Speech-Language Pathologist in Massachusetts.
- One year of experience working with adult/geriatric populations.
