Oak Street Health Inc is seeking a highly skilled Primary Care Physician Assistant to join our team of healthcare professionals. As a Nurse Practitioner in our organization, you will play a vital role in providing exceptional primary care to our patients.
Key Responsibilities- Provide exceptional primary care to patients, including routine and acute visits.
- Conduct office visits, administer annual wellness visits, and health risk assessments.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, in coordination with the Care Team.
- Assist the care team with phone triage and outreach.
- Educate patients on their health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license.
- US Work Authorization.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
- Passion for teamwork and the opportunity to collaborate cross-functionally.
- Desire to be a part of an innovative model focused on empirically-guided population health.
- Bilingual proficiency in applicable areas.
