About the Role:The Wound Care Coordinator RN will be responsible for managing the facility wound care program, including standards of care and practice related to wound, ostomy, and continence patient care needs. This role will also provide direct patient care and assistance to staff nurses, and act as a consultative service to affiliated healthcare agencies.
Key Responsibilities:- Develop and implement the facility wound care program in conjunction with the national standardization process.
- Establish standards of care, competencies, policies, and procedures in quality, cost-efficient, and effective wound care for all clinical staff.
- Act as primary consultant to Wound Care Clinicians and provide training and support to clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care.
- Evaluate all wounds upon admission and ongoing to determine treatment plans and provide early problem identification.
- Provide consultation and/or assessment on patients with pressure ulcers and consult on any wound that does not show measurable signs of healing within two weeks.
- Make recommendations to the physician for changes to wound care orders and provide evidence-based research support as needed.
- Review charts of patients with wounds and ensure that wound assessments, care plans, and treatments are clearly and correctly documented.
- Investigate all cases with adverse events related to wounds and participate in clinical outcome monitoring, follow-up, and agency performance improvement initiatives.
- Collaborate with Support Center Clinical Operations staff to maintain a cost-effective wound care formulary and evaluate products and cost-effectiveness for use in the hospital.
- Review and develop with all involved disciplines a plan for discharge and evaluate culture results and conference with physicians on the Plan of Care.
- Educate staff on products available for use in the hospital and educate families on care of patients.
- Participate in business development activities to promote Kindred Healthcare facilities as the provider of choice for wound care and involved in marketing the wound care program through outcomes and relationships with physicians.
Education: Degree from an accredited program leading to licensure as a Registered Nurse.
