We are seeking a skilled Physical Therapist to join our team at SunCrest OMNI Tallahassee, a part of LHC Group. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Evaluate and Assess Patient Needs: Conduct thorough assessments and evaluations of patient care needs related to functional status, mobility, endurance, and other medical conditions.
  • Develop and Implement Treatment Plans: Work with patients to develop personalized treatment plans, perform interventions to improve their well-being, and evaluate their progress.
  • Collaborate with Healthcare Team: Consult with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
  • Provide Education and Support: Educate patients and their families on treatment plans, functional limitations, and progress toward goals.
  • Document Patient Progress: Maintain accurate and detailed records of patient progress, including progress notes and discharge summaries.
Requirements
  • Licensure and Certification: Current Physical Therapy licensure in the state of Florida and current CPR certification.
  • Education and Experience: Bachelor's degree in Physical Therapy and relevant experience in home health or a related field.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
