Position Summary:

As a Licensed Practical Nurse (LPN), you will work under the supervision of a Registered Nurse and/or designated authority, adhering to state regulations and nursing practices. Your primary responsibility will be to deliver direct, high-quality nursing care in alignment with established protocols, procedures, and physician directives.

Key Responsibilities:

  1. Deliver direct patient care in accordance with care plans, physician orders, and nursing standards.
  2. Provide education to patients/residents based on their individual needs and care standards.
  3. Maintain accurate documentation of assessments, care provided, and patient observations in medical records.
  4. Develop, implement, assess, and revise individualized interdisciplinary care plans.
  5. Oversee and guide Medication Aides, ensuring appropriate responses to medication effects or interactions.
  6. Assist physicians during rounds, examinations, and treatment procedures as required.
  7. Train and evaluate subordinate staff, ensuring compliance with policies and regulations.
  8. Foster a collaborative and respectful atmosphere with team members, families, visitors, and volunteers.
  9. Conduct nursing rounds, liaising with medical staff, and communicating patient care needs during shift transitions.
  10. Support the admissions and referrals process, including pre-admission assessments.
  11. Uphold confidentiality of patient, resident, and employee information in accordance with privacy laws.
  12. Notify relevant parties of any changes in patient/resident conditions and initiate necessary follow-up actions.
  13. Report any suspicions or allegations of abuse, neglect, or misappropriation of property immediately.
  14. Participate in the admission, transfer, and discharge processes, ensuring all documentation is completed.
  15. Treat patients/residents, colleagues, and the public with dignity and respect at all times.
  16. Mentor new employees and agency staff, ensuring they operate within their scope of practice.
  17. Encourage and support patient/resident autonomy and personal choices.
  18. Address complaints or suggestions from patients/residents, colleagues, or the public effectively.
  19. Promote and maintain a clean and safe environment, addressing any equipment malfunctions promptly.
  20. Respond to medical and psychiatric emergencies, adhering to safety protocols.
  21. Engage in staff development and participate in educational programs.
  22. Contribute to Quality Assurance Performance Improvement (QAPI) initiatives.
  23. Represent Syra Health positively to community organizations and educational institutions.
  24. Maintain consistent attendance and perform additional duties as assigned.

Qualifications:

  • Diploma from an accredited nursing program.
  • Current LPN license from Nebraska or a compact state recognized by the Nebraska Board of Nursing.
  • Certification in basic life support/cardiopulmonary resuscitation (CPR) or completion of certification within the required timeframe.
  • Preferred experience in various healthcare settings, including acute care, rehabilitation, skilled nursing, and long-term care.
  • Preferred familiarity with Electronic Health Record systems.
  • Completion of pre-employment drug screening, criminal background check, and health review, including physical abilities assessment.
  • Annual health review completion.
  • Current CPR certification.
  • Covid Vaccination or appropriate declination/waiver.
