Case Manager

We are seeking a highly skilled Case Manager to join our team at the St. Louis County Department of Human Services. The successful candidate will be responsible for providing case management services to older adult customers and family caregivers residing in St. Louis County.
Key Responsibilities- Conduct gerontological assessments with older adult clients to identify individual needs and develop client-centered goals and action plans.
- Provide crisis intervention and support, assessing customer risk for harm to self or others.
- Assist customers in accessing services to address health needs, housing, nutrition, socialization, and specialized programs and services.
- Develop an assertive care team of partner staff and co-workers to provide well-integrated, effective, and highly valued services to customers.
- Identify common needs of current and new customer populations and arrange successful referrals to services and resources in the community.
- Bachelor's degree in Gerontology, Social Work, or a related field; Master's degree preferred.
- Three years of work experience related to providing case management services to older adults in a professional setting.
- Experience and skills in forming positive working relationships with adults of various cultural backgrounds.
- Computer literacy (MS Word, Excel) and strong verbal and written communication skills.
St. Louis County offers a comprehensive benefits package, including paid medical insurance, 20 days of PTO (paid time off) the first year and increasing with seniority, and 11 paid holidays. Employees hired for full-time employment will participate in the St. Louis County retirement plan, where they contribute 4% of their salary to the defined benefit plan with vesting after 7 years.
Additional benefits include paid life insurance, a credit union, 457 Deferred Compensation Plan, pre-tax premium pay plan, dependent care and medical expense reimbursement plans, and short and long-term disability insurance.
As of January 1, 2023, St. Louis County approved up to 480 hours of paid parental leave for eligible employees welcoming new children, either by birth or adoption.
