The role of a Licensed Practical Nurse (LPN) encompasses the execution of designated tasks and the sharing of responsibilities under the supervision of a Registered Nurse.
KEY RESPONSIBILITIES:
- Execute selected tasks and activities under the guidance of a Medical Doctor, Advanced Practice Registered Nurse, or Registered Nurse to gather and relay information that aids in assessing, planning, and evaluating patient requirements.
- Contribute to the formulation of the patient's care strategy.
- Engage in the delegation and provision of direct care, prioritizing needs and established objectives.
- Ensure a safe and health-promoting environment.
- Document nursing actions and patient responses to care.
- Communicate nursing actions and patient responses to relevant healthcare team members.
- Perform patient-related tasks, medication administration, diagnostic evaluations, and therapeutic interventions under the Provider's direction, ensuring safe and effective patient care delivery.
- Assist with medication renewals and prior authorizations as directed by the Provider.
- Manage triage phone calls and conduct follow-up calls post-surgery.
- Support and engage in quality improvement initiatives.
- Maintain accountability for professional skills and knowledge, demonstrating critical thinking to enhance patient care.
- Carry out additional duties as assigned.
- Valid LPN License in the state of Connecticut.
- Strong verbal and written communication skills, along with effective interpersonal abilities.
- Current CPR certification.
- Crisis Prevention Intervention (CPI) certification.
- Proficient in reading and writing English, with a solid command of spoken English.
- Ability to collaborate effectively with other healthcare team members.
- Commitment to maintaining patient confidentiality.
- Basic computer proficiency and ability to navigate internet and relevant software.
- Capability to perform automated tasks within job responsibilities.
- Ability to thrive in a time-sensitive environment, including exposure to various patient elements such as bloodborne pathogens; physical requirements include lifting and moving patients, pushing stretchers, standing, and walking.
- Potential exposure to environmental risks, including anesthetic gases.
- Ability to meet the care needs of diverse patient populations.
