Job DescriptionThe VNA RN Case Manager provides skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. This role involves utilizing nursing theory and processes, communicating/documents observations and assessments, and promoting and maintaining the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
Key Responsibilities:- Providing skilled care and case management to patients/families
- Utilizing nursing theory and processes to promote health and well-being
- Communicating and documenting observations and assessments
To be successful in this role, you will need to have:
- A Graduate degree from an approved, accredited school of professional nursing (BSN preferred)
- A current license to practice as a Registered Nurse in the Commonwealth of Massachusetts
- Minimum of one (1) year Medical/Surgical nursing experience
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills
