About the PositionWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes and other community settings.
Responsibilities- Conduct adult and geriatric assessments, medical history reviews, and provide health education to patients
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to deliver comprehensive care
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Work collaboratively with primary care physicians to provide follow-up care and education
- Master's degree in nursing or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health/medical record software
- Excellent verbal and written communication skills
Ability to travel, valid state driver's license, and proof of adequate automobile insurance coverage for the state of residence.
