We are seeking a dedicated and skilled Hospice Licensed Practical Nurse (LPN) to join our team at Interim HealthCare - Louisiana. As a Hospice LPN, you will provide high-quality, patient-centered care to patients with six months or less to live, focusing on quality of life.
Key Responsibilities- Provide in-home care to patients, working closely with an interdisciplinary team to ensure comfort, manage symptoms, and offer emotional support.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Take a holistic approach to care that engages the patient's mind, body, spirit, and family.
- Graduate of an accredited practical nursing program and active LPN license in Louisiana.
- Hospice LPN experience required.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.
