Role Summary
Home Care Providers are tasked with delivering essential services within the clients' residences. This role is hands-on, focusing on supporting clients according to the specifications outlined in their personalized care plans.
Key Responsibilities
Responsibilities encompass, but are not limited to, the following:
- Review client care plans to ensure a comprehensive understanding of their needs, including personal care, homemaking, and respite services.
- Travel to clients' homes to provide services exclusively in their residences.
- Accompany clients to approved appointments using third-party transportation services.
- Utilize the agency's application to accurately clock in and out for completed shifts.
- Document mileage in the agency application following established procedures.
- Verbally report and submit written documentation for any incidents or accidents.
- Manage shift timings to ensure punctual clocking out.
- Establish and maintain rapport with clients and their support networks.
- Deliver services with compassion and care.
- Maintain professional boundaries with clients and their support systems.
- Monitor and report any changes in clients' conditions.
- Complete annual paid training (available virtually or in person).
- Implement state-mandated reporting protocols.
- Communicate any changes in availability promptly to minimize impact on client services.
- Uphold client confidentiality at all times.
- Employ critical thinking skills to navigate a work environment with potential interruptions.
Qualifications
Applicants must meet the following requirements:
- Must be at least 18 years of age.
- Possess a valid California Driver's License.
- Have access to a vehicle with a valid auto insurance policy.
- Provide proof of eligibility to work in the U.S.
- Pass a background check.
- Provide yearly TB clearance.
- Obtain and maintain valid First Aid and CPR certifications.
- Exhibit a strong work ethic with a focus on achieving goals.
- Ability to effectively use a cell phone and related applications.
- Enjoy working in a one-on-one environment.
- Able to provide services in clients' residences.
- Must be available to work a minimum of 20 hours per week.
