Job Summary

Key Responsibilities
  • Assess and evaluate patients' physical therapy needs and develop individualized plans of care.
  • Provide physical therapy services, including exercises, stretches, and other interventions, to help patients achieve their goals.
  • Conduct home assessments to identify potential safety hazards and provide recommendations for modifications.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care.
  • Document patient progress and communicate with patients, families, and healthcare teams to ensure continuity of care.
  • Supervise and train Physical Therapy Assistants and Home Health Aides as needed.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the state of Texas.
  • At least two years of experience as a Physical Therapist, preferably in home health or community settings.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
