We are seeking a highly skilled Physical Therapist to join our team at Advanced Home Health Services. As a Physical Therapist, you will be responsible for providing physical therapy services to patients in their homes, promoting independence and improving their quality of life.
Key Responsibilities- Assess and evaluate patients' physical therapy needs and develop individualized plans of care.
- Provide physical therapy services, including exercises, stretches, and other interventions, to help patients achieve their goals.
- Conduct home assessments to identify potential safety hazards and provide recommendations for modifications.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care.
- Document patient progress and communicate with patients, families, and healthcare teams to ensure continuity of care.
- Supervise and train Physical Therapy Assistants and Home Health Aides as needed.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist in the state of Texas.
- At least two years of experience as a Physical Therapist, preferably in home health or community settings.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
