Responsibilities
- Assess and Plan Patient Care
- Utilize the nursing process to identify patient needs and develop individualized care plans.
- Conduct thorough assessments to identify patient strengths, weaknesses, and potential health risks.
- Implement and Evaluate Patient Care
- Develop and implement patient care plans in collaboration with healthcare professionals.
- Evaluate patient progress and adjust care plans as needed.
- Collaborate with Healthcare Professionals
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss patient care and share best practices.
- Document Patient Care

Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of practice.
