Position Overview:
The RN Care Coordinator will oversee all systems and services necessary for a structured, multidisciplinary, patient-centered care team approach, ensuring quality and cost-effective care for the designated patient population. This role involves managing patient treatment plans by coordinating care with physicians, nurses, and other healthcare staff, as well as community health providers, to achieve optimal patient outcomes within set timelines and efficient resource utilization. The RN will liaise with community providers, insurance companies, and agencies throughout the care continuum and will supervise the work of LPN Care Coordinators. Participation in Patient-Centered Medical Home initiatives is also required.
Minimum Qualifications:
Active RN Licensure in New York; Associate's Degree in Nursing; Bachelor's Degree in Nursing (BSN) preferred; Certification in Case Management (CCM, ACM) preferred; Familiarity with the provider community and resources in the Syracuse NY/CNY areas; Self-motivated, enthusiastic, and innovative; Strong communication skills for outreach to community healthcare organizations; Ability to maintain confidentiality in accordance with Practice, State, and Federal regulations; Demonstrated sound judgment, decision-making, and problem-solving abilities; Highly organized with well-developed oral and written communication skills; Comprehensive understanding of insurance principles and outpatient delivery systems.
Preferred Qualifications:
A minimum of 5 years of clinical experience.
This position is remote; however, candidates must reside in Central New York.
Compensation Range: $58,000 - $72,000 based on experience.
Work Type: Full-Time
Work Hours: 8:00 AM - 4:00 PM
