We are seeking a skilled Registered Nurse to join our Hospice Care team at Henry Ford Health System. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families in a compassionate and respectful manner.
Key Responsibilities- Provide nursing care to hospice patients in accordance with the hospice concept of care.
- Assume managerial responsibilities in the absence of the Hospice manager.
- Work under the supervision of the Hospice manager.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support.
- Provide education and support to patients and their families on end-of-life care and related topics.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Graduation from an accredited school of nursing.
- Current Michigan licensure as a Registered Nurse.
- Hospice certification preferred.
- A working knowledge of current pain and symptom management, and experience appropriate to the needs of hospice patients and their families.
- Interpersonal skills to present a positive image of Hospice to those whom are referred to the program.
- Ability to be flexible in adapting policies and procedures to meet the needs presented by Hospice patients and families.
- Valid drivers license, acceptable driving record, and reliable transportation.
