- Conduct nursing assessments and collaborate with the healthcare team, patients, and families to formulate effective care plans.
- Set realistic, measurable short- and long-term goals for patients, implementing interventions as necessary to reduce rehospitalizations.
- Administer medications and perform treatments as directed by physicians.
- Delegate patient care tasks to staff and supervise their execution to ensure quality care.
- Facilitate communication of patient information among staff and across shifts.
