**Job Summary**
CodeForce is seeking a highly skilled Care Coordinator - Utilization Management and Discharge Planning Specialist to join our team. As a key member of our care coordination team, you will be responsible for ensuring seamless transitions of care for our patients, while also optimizing resource utilization and promoting efficient care delivery.
**Responsibilities**
* Utilization Management: Conduct thorough reviews of patient records to identify opportunities for cost savings and quality improvement, while ensuring compliance with regulatory requirements.
* Discharge Planning: Collaborate with interdisciplinary teams to develop and implement comprehensive discharge plans, ensuring timely and safe transitions of care.
* Communication: Build strong relationships with patients, families, and healthcare providers to ensure effective communication and coordination of care.
* Data Analysis: Utilize data analytics to identify trends and opportunities for improvement, informing strategic decisions to optimize care delivery and resource utilization.
**Requirements**
* Current Massachusetts Registered Nurse License
* Previous experience in utilization review and case management
* Strong communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
* Proficiency in Meditech Expanse and InterQual
**Preferred Qualifications**
* Experience with discharge planning and care coordination
* Knowledge of regulatory requirements and industry standards
* Strong analytical and problem-solving skills
* Ability to work independently and as part of a team
