Position: Nurse Practitioner - Advanced Practice Provider

Overview: The Nurse Practitioner role at Oak Street Health is designed to deliver high-quality, equitable primary care to adults, particularly those on Medicare, ensuring their well-being and minimizing hospital visits. Nurse Practitioners operate autonomously while also collaborating with physicians based on individual patient needs. They foster long-term patient relationships, which cultivates trust and enhances patient understanding of their care plans.

Team Structure: Nurse Practitioners work within outpatient facilities as part of a collaborative care team that includes physicians, other nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We collaborate with a network of distinguished specialists and hospitals for additional care services, allowing our healthcare providers to concentrate on primary care, care coordination, and population health initiatives.

Key Responsibilities:
  • Deliver exceptional primary care services.
  • Conduct routine and urgent office consultations.
  • Perform annual wellness evaluations and health risk assessments, emphasizing a comprehensive approach to health and meticulous documentation.
  • Coordinate care with other healthcare providers, specialists, diagnostic facilities, and agencies.
  • Lead population health initiatives in collaboration with the Care Team (e.g., ensuring all eligible patients receive necessary screenings).
  • Assist the care team with telephonic triage and outreach efforts.
  • Educate patients regarding their health conditions, treatment plans, and care strategies.
  • Engage in promotional activities for Oak Street Health.
  • Conduct home visits when necessary.
  • Other responsibilities as assigned.

This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications:
  • Master's degree in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
  • National certification in at least one of the following areas:
    • Family Nurse Practitioner
    • Adult-Gerontology Primary Care Nurse Practitioner
    • Adult Nurse Practitioner
    • Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA registration.
  • Authorization to work in the United States.

Preferred Qualifications:
  • Experience in primary care settings such as internal medicine, geriatrics, or family medicine.
  • Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
  • A strong commitment to teamwork and collaboration across functions.
  • A desire to contribute to an innovative model focused on evidence-based population health.
  • Bilingual skills in relevant languages.
