About the Role:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver personalized care and support.
Key Responsibilities:
- Provide home-based care to patients with illnesses, injuries, and chronic diseases, under the supervision of a Registered Nurse (RN)
- Work collaboratively with a team of healthcare professionals, including physicians, RNs, Certified Nursing Assistants (CNAs), and home health aides, to deliver comprehensive patient care
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
- Assist the RN in carrying out the patient's plan of care and teaching self-care techniques
- Educate family members on the patient's plan of care, medication, and home safety
- Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge
Requirements:
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state of South Carolina
- At least one year or 1200 hours of experience as an LPN/LVN
