Job Summary

As a Licensed Practical Nurse at Physicians Dialysis, you will play a vital role in coordinating, implementing, and documenting patient care for individuals undergoing dialysis treatment. Your expertise will be essential in ensuring the highest level of care and comfort for our patients.

Key Responsibilities
  • Pre, Intra, and Post Dialysis Assessments: Perform thorough physical assessments of patients before, during, and after dialysis treatment, documenting any changes or concerns.
  • Dialysis Treatment Management: Initiate, monitor, and terminate dialysis treatments according to clinic policies and procedures.
  • Medication Administration: Administer medications as prescribed, documenting administration and results.
  • Hemodialyzer and Machine Preparation: Prepare and monitor hemodialyzers and machines, performing quality control checks as required.
  • Lab Specimen Collection: Obtain pre, intra, and post laboratory specimens as ordered.
  • Lab Procedure Performance: Perform in-house laboratory procedures in accordance with clinic policies and Federal and State laws.
  • Lab Report Review: Review lab reports, identify problems, and report to Charge Nurse and Registered Nurse as required.
  • Nursing Assessments: Complete monthly nursing assessments and assist in reviewing and co-signing assessments performed by non-licensed personnel.
  • Patient Education: Review and document patient education to ensure compliance with ESRD Network, regulatory agencies, and the facility's CQI program.
  • Consent Forms and Policies: Review and obtain signatures on consent forms and facility policies upon patient admission and annually.
  • Transfusion Monitoring: Prepare and perform intra and post transfusion patient monitoring.
  • Quality Control and Safety: Complete quality control checks for emergency carts and kits as assigned.
  • Administrative and Staff Support: Participate in administrative and staff programs, including staff meetings, company in-services, and CQI meetings.
  • Order Transcription and Implementation: Transcribe and implement physician orders.
  • Emergency Response: Recognize and respond to emergency situations.
  • Staff Training and Development: Assist in teaching and training new staff members as directed by Charge Nurse, Registered Nurse, or Nurse Manager.
Supervisory Responsibilities
  • Unlicensed Personnel Supervision: Oversee the overall direction, coordination, and evaluation of unlicensed personnel assigned to your area.
  • Supervisory Responsibilities: Carry out supervisory responsibilities in accordance with facility policies and procedures and applicable law.
Competencies
  • Nursing Management: Apply practical knowledge of healthcare concepts, practices, and specialized techniques related to nursing management.
Required Licenses/Certifications
  • Licensed Practical Nurse: Credentialed from the State Board of Nursing in the state where the site is located or licensed in a compact licensure state.
  • CPR Certification: Current CPR certification. CCHD certification is encouraged.
Education/Experience/Qualifications
  • LPN Program Completion: Completion from an accredited Licensed Practical Nurse program.
  • Orientation and Training: Must complete the facility's orientation and training program.
  • Clinical and Hemodialysis Experience: Must have at least 2 years of clinical and hemodialysis experience.
  • Commitment to Safety and Excellence: Must have a demonstrated record of and commitment to safety, and dedication to excellence.
  • Communication and Interpersonal Skills: Must possess outstanding communication and interpersonal skills, able to communicate professionally and effectively with other staff members, patients, and physicians.
  • Computer Skills: Must have computer skills, including data entry, navigation around software, and basic troubleshooting skills.
