Job Description

The Pediatric Care Coordinator is responsible for providing clinically and evidenced-based patient care coordination, supporting the delivery of safe, effective, high-quality, and efficient patient care at Children's Healthcare of Atlanta. The coordinator will assess, plan, implement, and evaluate patient care to ensure smooth transitions from hospital to home or other care settings.

The ideal candidate will have 3 years' experience with a Bachelor of Science in Nursing (BSN) or a current Children's nurse with 5 years' experience with an Associate's degree in Nursing. A Master's degree in a related field is preferred.

The Clinical Patient Advocate will cultivate and maintain effective interaction/communication with members of medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care.
