We are seeking a dedicated Care Transition Pharmacist to join our team in a healthcare environment. This role is essential for supporting patients with complex medical needs as they move from hospital care to their homes and communities.
Compensation: $65-$68 per hour, based on experience
Location: Sylmar, CA
Work Schedule: Monday to Friday
Employment Type: Full-Time, Temp-to-Hire, Direct Hire
Benefits: Comprehensive benefits package including Medical, Rx, Vision, Dental, and 401(k)
Key Responsibilities:- Assist high-risk patients with medication management during their transition from inpatient care.
- Identify and resolve medication-related issues to enhance patient safety and reduce readmission rates.
- Provide pharmacist-led interventions and patient education regarding medication.
- Collaborate within a pharmacy program focused on clinical care for vulnerable populations.
- Must possess a valid California Pharmacist license.
- A minimum of 1 year of experience as a Pharmacist is required.
- Doctor of Pharmacy (Pharm D) degree is mandatory.
- Completion of a PGY1 residency is preferred.
- Must be at least 18 years old.
- Background check and drug screening are mandatory.
- Health screening is required.
- Proof of eligibility to work in the US is necessary.
